Frequently Asked Questions

We provide the direct case management services required for each individual client to access other assessed needs to remain safely in the community. We assist in coordinating a safe discharge from a skilled nursing facility back to the community, and ongoing case management long term.

We are accepting new Medicaid patients. We do not accept CHIP patients.
*Please ensure that there is an open Medicaid case for the patient and keep it open/active in order to receive our services.

We do provide Telehealth services when appropriate.

We do not provide services within our office setting- we come to the client and offer services within their setting. We can assess and explore options for ADA accommodations as needed for each individual client.
